Obviously it isn't uncommon for bands and fans to get pissed at each other, especially at shows. And sometimes these incidents become infamous.

Fleetwood Mac are performing "Beautiful Child" and a fan comes on the stage (it's at about 4:30) http://www.youtube.com/watch?v=vigBfdMs2y4&feature=related and he puts his arm around Stevie and they just keep performing as if nothing happened (listen to her comment at the end)

BQ: I like the Pearl Jam one because he was just so chill about it and he made the audience laugh even though some people were being shitty

BQ2: Sometimes it is because they should expect stuff to happen but at the same time I can't blame them because they are trying to perform and people are being assholes.

BQ3: I have no idea. I don't know which bands flip out a lot. But I know Pink Floyd did the whole wall thing at their concert because it was to do with the fan interaction and how the fans would just scream and yell and such instead of actually caring and listening to the music.

Enslaved all the way... Also check out Absu and Wolves in the Throne Room. Absu feels to me like Blackened thrash at times and WITTR is VERY proggy black metal. Listen to a wide selection from their catalog to get the best idea about who they are.
