It's the foot that corresponds to your dominant hand. If you're right handed, as most are, you'd generally use your right foot.
The bass drum is the big one on the floor, at least with one exception that I've seen. I saw this one band where the drummer actually had an elevated floor tom that was huge and they played that by hand in place of a kick. They weren't a very good band though, I can't remember the drumming specifically (except that it was odd that the drummer did it that way), but the band wasn't very good.

Usually it's the right, but if you are playing double-bass drums, it's both, obviously. I've seen left-legged drummers (Tony Royster, Thomas Haake).

Most drummers actually use both legs and have two bass drum pedals or two separate bass drums.

Usually the right, and the left is on the high hat. Unless it is a double bass, then it's both with the left alternating on the high hat.

It doesn't HAVE to be the right leg. Really which ever leg you want, that's why drums are modular so you can position them anyway you like.

I can't believe people here are just saying "right" as if it's a standard. The truth is, either. If the drummer is right handed/footed, then he'll have the bass pedal on his right. If he's a lefty, then usually the left foot plays the bass drum and the whole set is "mirrored" from the way a right hander would play. Jane's Addiction's drummer is a lefty.
Here's a trick i tell all my younger students.
-Stand up straight.
-Lean forward until you're about to fall over forwards.
-whichever foot stops you from falling is the foot that you'll use to play the bass pedal.
